The Support Coordinator LOCATES,COORDINATES and MONITORS service for people with DISABILITIES.
The Supports Coordinator will engage individuals and their loved ones in the development of plans that assure the needs of the consumers are addressed and life opportunities expanded, assist in the choice of services and supports, coordinate the provision of services and supports, and monitor the delivery of services and supports.
The candidate will promote individuality, choice, and control. The position will be performed consistently with the values, principles, and philosophy of everyday lives.
The candidate will have a strong belief that people with Intellectual Disabilities can achieve desired outcomes and be valued and can contribute to their community.
The person must be proficient in the use of computers and have transportation.
Bilingual a plus!!!
Job Type: Independent Contractor positions
Required experience: working with persons with intellectual disabilities : 1 year
Required education: Bachelor's Degree in Human Services field
